Oh, I see: a fat man suddenly dies from heart attack. The food in his hand suddenly transforms into a skull, showing again that eating too much is a cause. The toppled hourglass and candle both symbolize the abrupt end of the life, while the remaining sand and candle length show how that he was intended to live approximately twice more. Or, we may hope, that his life still can be saved by the medicine, candle re-lit and hourglass put upright again.
