Actually, it's an AU where Sans got a hold of Frisk's soul during a genocide route and found Frisk and Chara fighting there. He uses Frisk's power to do a true reset, but because of this Sans ends up in Frisk's body. And Frisk is sitting there in Sans mind like Chara used to sit in their's (if you notice the tiny nervous soul floating in front of their chest?)
