I like the idea of using a shrunken person as a toothbrush by a woman, but it's not clear if it's a male. Yes, it's not easy to represent an image where a shrunken man is brushed against woman's teeth and show his sex, but in that way it's not clear if the idea is of a male serving a woman. I like women smiles so I appreciate the idea for his originalty since I don't remember having seen something like it anywhere.
Speaking of the quality level from a technical point of view it's really good: I must say that the details of her skin are excellent. Only that foam should maybe mixed with a little of blood; but I understand your choice because you did not mean to represent a kind of hard-vore picture even if a little of blood could be present.
I suggest to change the point of view to improve the impact, too. If the camera is a little moved to the left and rotated 30° to the right (or to the left) a better effect to the image would be added.
In the end it's a fantastic picture, but that could be better represented. I hope you'll do other pictures like this changing the point of view and maybe the woman who's brushing her teeth.
