It's a part of Misdreavus's body. It's how it gets nutrients; the beads absorb fear, which it feeds off. That's just how Misdreavus is. They aren't removable.

There's been no evidence thus far that they are. Every bit of information has pointed to the fact that Misdreavus' beads are necklace-like but not an actual necklace.
