Read the Rules and the information below before using this sheet.

1. Try and keep the forms Appropriate/Child-Friendly, We don't want to see some yucky stuff from this sheet
2. You can use blood on one of the forms, but just a little bit please, I don't want too much gore
3. Feel free to edit the sheet as long if its Appropriate/Child-Friendly and sticks to the alternative appearances/forms theme.
4. If you want to use a character from a tv show/film/game, make sure you put the name of a TV/Film/Game and it's company name (E.g. Insektors (C) Fantome)
5. If you want to use your OC, Go right ahead!
6. Once your done with the sheet, link back your character's alternative appearances and forms on the comment section on this page
7. Have Fun!

Normal: Basically, this is your normal form. Basic, unchanged and perfectly normal person.

Superhero: Every Superhero needs a super power, your powers either have element powers, telekinesis, super speed, etc. You could always make your own super power if you want, oh! and don't forget your very own superhero costume

Robot: Your robotic form could have any design it pleases, such as scanning humans, flowers, animals, etc. or have wheels for legs or an automatic jetpack or maybe some cool build in weapons for combat.

Evil: Most commonly, there are some evil versions of the heroes characters in most of their games, but should you be worried of what your evil self would look like if you met them, (Note: If you already have an evil main character, it would be a Good version of yourself in the evil universe)

Were-Animal: Your character doesn't have to be a were-wolf, it could be any kind of were animal, Were-Cat, Were-Mouse, Were-Rabbit, Were-Bear, etc.

Kaiju: Means Strange Beast in Japan. Kaijus are known as mutated giant monsters like Godzilla and Mothra for an example. Your kaiju form can also be from a non-Japanese film but it still needs to be from a giant monster film.

Vampire: Yeah there are many different vampires with many different looks. They could look gothic, elegant, steampunkish, etc.

Insect: If everyone on the planet was transform into those small bugs, picture yourself as a colourful or dull insect, Could be a butterflies or A Stag beetles or cockroaches and even spiders are welcomed is well.

Dragon: Imagine yourself as fire breathing dragon or a ice breathing dragon, which ever way works.

Gender-Swapped: From a different dimension, what does your Gender-Swapped version look like?

Medieval: Travel back to the past and don some cool Medieval clothes (Medieval RPG games counts is well) you can use any weapon but it has to be part of your Medieval form.

Other: If you have a form that wasn't featured on one of the squares, you could always add the form you want on the others square, but can be anything that wasn't on the sheet, such as a futuristic form or a fairy form. But like I said you can always edit the sheet to change the forms names as long it's child-friendly/appropriate.
