Don't know. It's just what I gathered by looking at the design and remembering what Goner Kid and the Gaster Followers look like. As for being a girl? In the game, Monster Kid had they pronouns like Frisk and Chara, so the same can be said about Goner Kid. They can be girls if we the artist wants them to be.
