Depends on the suspension...
If it's rope, depending on the thickness, material and weave of the rope, it can be fairly... not really comfortable but at least tolerable.
If it's hooks, chains or other types of suspension that hardcore fetishists use... I could imagine that being incredibly painful.

Personally I've always found suspension very painful, partially because of the disruption to the circulation system, partially because of nerve compression, partially because of intense pressure on a few isolated parts of the body; and it's often very disorientating.
I've never done a body hook suspension, not really my thing.
It's really not that painful at all. Pinch your nipple with your fingers - slightly more painful than that but not very.

I've never done much in the way of.... well, for lack of a better phrase, deviant fetishes, so I wouldn't have much to compare to. The closest to suspension I've done is climbing trees and hanging from ropes during military training and that was fully clothed.
