I believe it was ketchup as well. I mean, imagine Papyrus if he bled like everyone thinks Sans does. I mean, everything points to Sans surviving the strike from Frisk. Maybe when he says that "You really won't like what happens next." He isn't referring to himself, but rather the encounter between frisk and chara.

I doubt he survived it, when he went offscreen you heard the monster death/turning to dust sound effect and leveled up, meaning you got the exp from killing him.
