meh, it's late...What you have here is a front & side template of a product box. Read the tips inside to use it properly. Then just play the attached action and get a nice looking 3d product box (no manual transformations needed). Originally made with Photoshop CS4, not sure if it's backward compatible (will not be compatible with anything lower than the first CS version because of the layer structure).
Also, check the "options" groups for different lighting options (if you don't like any then just create one yourself, no problem to apply it). I've left it optional because sometimes you may need to simulate a specific surface with a tad different light refraction.
Anyway, i hope it'll be useful to you guys.

It turns out this template is also quite rescalable. You can change the size of sides by editing the folder mask. HOWEVER, it may (and highly likely will) get a bit funky with the reflection - you'll possibly lose it. If you don't need it anyway, feel free to edit the sizes (on occasion it might require additional editing limited to gentle evening out the height).
Minor issue
It seems there is a slight problem with localized versions of Photoshop. The action will stop because copied layer may have a different name (for example, in Polish it will end with "kopia" rather than English "copy"). Fast solution is to change the layer name accordingly to "*LayerName* copy" and resume the action. Sorry for the inconvenience, I'll try to fix it in the next release.

Hehe, thanks. Though the design isn't the case here, rather the action which generates the supposed 3d effect (its highly outdated, given the intelligent layers feature in hmm... 3-4 latest versions of Photoshop).
