Now I would have liked for this to end a little different with more poetic justice. Let his wish come true. Let his friend get the mind of a sexy, wild and confident girl, but let her be so sexy, wild and confident that she realises that she is far above his league, dumping him on the spot to go find hotter guys.

Well, for one, imagine a guy is constantly getting hounded for not focusing on his school work, so they send a strict tutor to him. In truth, the reason why he's been sluffing off is due to studying magic hypnosis. When she wants to get started with him, he acts like "Let's get this over with then. My hardest part I'm having trouble with is in that book on page (insert page number)." She turns it there, and finds a hypnosis spiral with runes, along with a picture of an ditzy over-developed anime cheerleader, arrows pointing out details on her. The tutor then changes physically, mentally, and even her clothes change. She snaps out of it after changing: "Like, what was I, like, doin' here again? I know it was to teach somethin', but me no know much smart stuff." He suggests that she was there to teach him stuff in bed or somethin' similar. You can guess the results.

You have many questions and wonders and I can answer all of them in this comment. It's a TG comic, things never have to make sense nor need logic. I gave it a basic story so it made for a more constructive TG than just walking down the road and suddenly BAM, they turn into a girl for no real reason (and I have seen a tg as simple as that too). But for the sake of answering your questions here goes.
page 1:
Sadly I have met people as shallow as carl and they think it's okay to still be friends with them.
Page 2:
As I said before, this is a very basic story with no real meaning to it but there is a plot to get you started in the page 1 description.
Page 3:
The guy is basically a douche-bag as you may have already guessed.
Page 4:
If i was changing into something else, I'd be too shocked and terrified to think straight. Plus Sam has no real idea how he is doing this as Carl said that the necklace worked under his breath.
Page 6:
Again, shock and confusion and oblivious to how he is doing this.
Page 7:
The woman most likely told him that the amulet only had so many wishes but figured she was talking rubbish but after seeing that it worked, he knew that the woman's words were true. Also, the amulet had shown clear sign that it had no more power left in it as it stopped shining. Finally, the comment at the end was really more for us than her as it would make you think "oh damn, whatta douche bag".
Remember, there are thousands of TG's out there that have less of a story than mine so atleast I put a little thought into it instead of a random sudden change of gender.

Fair enough, though I still don't agree with your contention that TGTFs have to be entirely divorced from reality, it's only the change itself, not the character's behavior that should classify as unreal.
