Hello, first let me say I love your work. Regarding this piece I like how the story fits in with this picture and wouldn't change the messages you are trying to get across at all, except that from reading this story it seems like the menstrual cycle did indeed help her personality stabilize since it seemed the most stable at the end of the story - as such in my revision I omit talking about the menstrual cycle entirely.
There are a few uses of present tenses that I think would be better as past tenses and vice versa. Also the way it is written, there are just a couple turns of phrase that I would change. Below is going to be a whole rewrite showing how I would revise this. I hope it is helpful.
She turned around and planted a deep kiss into the boy behind her, so happy she had found someone who truly understood her struggle. It had been years since she had transformed into a girl. She revolted against her feminine form many times, dressing as a guy and trying to reject her current reality. Her new body was an insult to her masculine pride and a constant reminder of how he became trapped in this body; an accident in the gene therapy lab. It took him three weeks to regain consciousness and he awoke to this new feminine form and the knowledge that the lab, and any chance of return to masculinity, was destroyed in the accident.
His new body became his prison and sent him spiraling into a depression that lasted weeks. His parents and doctors sent him back to school; it was the only way they believed he could adjust to his new social conventions and develop into womanhood. For him it was a long, dark battle between his masculine mind and an emerging feminine self - a feminine self that was gaining control. As she learned and lived as a woman, he withdrew from this new self. As she became more comfortable with her life, her old self became more lenient and flexible. Things weren't so bad, he had just been angry that his identity was stolen from him by an odd turn of fate. But stolen wasn't quite right, no, it had been more... refreshed.
Finally, after four years of fighting, struggling, anger, depression, and confusion she accepted who she was as she embraced the boy behind her. She had a crush on him for some time and the embrace felt right. Had her hormones altered her thoughts? Probably, but here she was now - happy and content with her life.

Love the image! It is extremely romantic and I recognize it. A lot of m2f stories skip the annoyance of periods. I can only imagine the mental chaos from losing power over both your identity and your emotions. Puberty must be extremely stressful for girls.
