You know, if you want to pull a bit more traditional, I wouldn't be too surprised if April came in time that they were leaving and Casey believes that April saved him, but not only does he have a hunch that he's wrong, but Donnie falls in love with her and Raph wouldn't be the only one that wants to go to the surface. XD

About that..
I feel that eventhough they live in a sewer, Mikey wouldn't be so uneducated about heartbeat as opposed to Scuttle and Ariel who both REALLY don't know much about humans. Mikey must've at least learned this from his family or the cartoon shows he watches. So I feel it'd be out of character for him to start checking heartbeat on the foot.....
Furthermore, if I put Mikey there and he starts checking Casey's foot, the first reaction from Raph would be to pound Mikey's head. That would break the tension a bit and the mood imo.
Don't worry, Mikey still have other roles to play in the story.
