Natani probably just whispered to Flora that he had breasts, and Flora made her own assumptions about their size. Natani didn't say anything about not being a boy, because in the current comic he still likes being seen as a man, no matter what his body looks like.
This is coming from a real life man who had boobs.

Flora was that way about calling Natani a boy because it had been discussed earlier in the comic, with her present, that yes Natani has boobs but that he prefers to be known as male.Β Hope that makes sense.
