If he's included in any way in 4e (he's mentioned in Open Grave, but that's all I've found so far), it won't be in this form.
By 2e he'd already surpassed his need for that form and had ascended into demilichdom. By the end of 2e he'd been destroyed in the Return to the Tomb of Horrors module, leaving nothing but a vestige trapped between the planes (3.5 Tome of Magic).
Excellent image, though. Really captures him before his ascension.

Also, my bad. Open Grave includes stats for Acererak as a demilich for those who wish to update either Tomb of Horrors module to 4e.

They're called ioun stones and they give various magical bonuses.
Acererak is a bastard. I put my gaming group through his Tomb of Horrors once and it nearly broke up on me!
This is a pretty damned good depiction of the bastard though
