Warning : This story's an alternate universe and is really crazy as hell. It crazy until crazy word isn't fit it anymore. It's become chaos now...
Warning : a lot of 4th wall breaking and referencing. Sharing universe with "To Love God - To Love Mortal" and "To Love Alicorn" but in the future and more crazy. And a continue story of "Crazy Future"
Warning : Don't take this story too serious or you might get headache from overthinking....

actually, the Title "Dark Souls" comes from the fact that Humanity (an ingame item that helps remove hollowing) is a Black Soul. Not only that, but, you use Souls to lvl up and increase your power and buy items. You're not on a timer, you can die as many times as you want or need to. The thing that makes the game hard is figuring out where everything is without opening yourself to being attacked by surprise or ganged up on so you don't die and potentially lose your souls.
Demon's souls in my opinion actually does a better explanation on why souls are so important compared to Dark souls. While it's not the better game, the lore is more coherent then Dark souls.

There a shrine where you offer soudls to reverse Hollowing, but that's in Dark Souls 3.

Actually, there's the Humanity Mechanic from all the games that stop the Hollowing Progress, which I suspect is offering Human souls to a campfire to Reverse Hollowing, as well as 'Solid' Humanity that reverts them to normal. However, it'll still happen over time and I suspect it can be accelerated in certain situations.
However, there are some that can still remain Lucid if they have something other than fighting to focus on, like Blacksmith Lenigrast, who still remained sane even though he's Hollowed by focusing on his craft and Daughter.

Yes and think costs more and more souls to reverse it. Whih makes it difficult, because there always that one moment of bad luck and BAM you died and lost the souls, or try to go back and get killed again and lose them.

Well, while you do lose souls per death, you don't lose items. And 'Solid' Humanity is considered an Item. That means when you are reborn, you can just pop a Humanity 'pill' and you'll restore yourself to normal. In fact, I think a good strategy is to stockpile 'Solid' Humanity by 'Farming' certain enemies until you have enough.
However, I suspect that AJ's Hubby didn't stockpile enough 'Solid' Humanity and thus kept loosing more and more of his humanity until there was nothing left, thus Forcing AJ to kill him.
