In the earlier issues, The Leader did some stuff to Hulk which turned him in to a scrawny weakling. And when his old enemies, the U-Foes began beating him to death, the Joe Fixit Persona took over, and enraged by how his alter ego was treated, Joe managed to trigger a transformation in to a Red Hulk inside of the fractured psyke. Which would also resort in a transformation back in the real world.
