I like the contrast. you have the buff model looking serious and the little kid trying to look serious, but you can tell by the way his feet are curled upward that he isn't so sure of the whole thing. Pretty cute.

It is only disturbing if you think of it as "...a kid beside this hunky sexualized model." While everyone else sees it as a before-after or a young-old, you are the only one to see this as a sexual thing. So you sir, are the disturbing one.

Well when you think about it, or not really, it's quite obvious, you'll see they're put in classically powerful positions, and the model has no shirt, the faces, the lighting; it all leads to a sort of sexual vibe... creepy.
I'm not saying it's on purpose, it probably just stems from the majority of the artist's work, very sexual photographs.
