I notice the printed page and this have Kaine in different versions of his outfit. At what point did this happen, and how frequently do things like that pop up?

You're right. Β I didn't even notice the comic until you mentioned it. Β Looks like the costume changed on page 8, where they were at the Jean Grey School. Β I do remember Carlo Barberi saying the costume needed to be changed in mid story. Β He must have done it on his end.
