The English major in me can see your illness represented in their misery, with an unclear conclusion meant to show your agony of feeling like you will never get healthy.

Theory:
That's the afterlife. Sam has been lying there sad and cold for many years, and when Digger shows up, he's both happy and sad. He's happy to have his friend and lover back, but he's sad that Digger's life was cut short.
Wow, imagine how he'll feel when Digger has to leave again to go to the White Light...
