Black Lady, age 18-20(Darker Pink hair): Rini's evil form after she came home to the future with Sailor Moon and the others to try and awaken her mother, Neo Queen Serenity. When it failed, she ran away feeling frustrated even after her efforts. Rini was captured outside the palace by Wiseman and brainwashed into thinking no one cared for her. It should be noted that unlike the first anime version of Sailor Moon R when she considered Luna P her only friend and corrupted it with her new powers, Black Lady tosses it aside by deriding it as nothing more than a child's toy. How she finally breaks free from Wiseman's control is different. In the Manga, it was Sailor Pluto's sacrifice by using Time Stop in preventing the Silver Crystal and Black Crystals from merging into something destructive. Before dying, she blamed herself in not protecting Rini, which allowed her to apologize for stealing a time key from Sailor Pluto that breaks Wiseman's spell over her. In the Anime, Sailor Moon risked her powers to become Neo Queen Serenity to show her true memories which she realizes that she is loved and returns to her original form.

Princess Lady Serenity, age 19-21(lighter pink hair): Rini's princess form when she needs extra powers, similar to her parents in their forms, when her Senshi or civilian form don't allow. She wears this during royal gatherings. In NSG, Rini attends Princess Training Academy which takes 5 years.

Queen Lady Serenity, Age 26-29(White hair with pink highlights): Rini inherits Crystal Tokyo from her mother and her new title is changed to Queen Lady Serenity. After the crystal seemingly disappeared, peace reigned for centuries. It's rumored that NQS has become the mysterious and elusive Sailor Cosmos.
