Axcul as we know is a nobody who's title is referred as The Reaper's Blade and this is why

Axcul willingly sacrificed his heart to revive his brother Lexaeus. The heartless that ripped his heart out by force and shattered it seconds later was a minion of The Reaper

Axcul's body as it faded into darkness was confronted by The Reaper and he planned on turning Axcul into a new minion but his will proved to strong for The Reaper to control so it offered Axcul a deal, "Become my blade and I'll give you a second chance at life to be with your beloved brother~" Axcul accepted the offer without hesitation and was reborn a nobody.

Little did Axcul know though was The Reaper's offer was also a means of him attaining a physical host to enter the real world since The Reaper(both The Heartless version(not the Port Rpyal one) and Nobody version(Axcul's Reaper) are sealed beings because they were considered dangerous.

Axcul has a degenerative mind, meaning he's slowly losing his sanity. This is because he has to forcefully suppress The Reaper from taking control of his body on occasion since it has a mind of its own it will occasionally not obey Axcul and do as IT pleases.

When The Reaper slowly breaks through for control Axcul's simplistic changes are his calm demeanor becoming more psychotic and bloodthirsty and his blue eyes glow and turn blood red, but Axcul is still the one in control. The Reaper truly has control when the white's in his eye turn black and his eyes are still blood red similar to how a vizard's eyes from BLEACH change only instead of yellow Axcul's are blood red and he lacks a mask.

The Reaper when in partial control physically manifests by absorbing Axcul's body and at first appears like and armorless nobody with a blood red scarf which is blood red because its actually bathed in blood for its scarf ends are sharp as blades and its main means of attack. Chains also manifest from the scarf to restrain and hold down Axcul.

The Reaper is a deadly force...after all it is DEATH IN ITS TRUEST FORM.

So we based The Reaper's form on Thanatos of Persona 3
