it's a lost soul head, like the ones you can see in the asriel fight. it's there because no one can remember what he looks like. his soul was shattered across space and time, so his soul is 'lost.' everyone else sees his normal head, though. the comics where his normal head is shown is just because the joke only works if you can see his face or something, it's basically just an appearance thing.

-I walk over and see the red dot, my pupils turning to slivers- (it does that when my OC Winters cat-like instincts kick in)

-I chase the dot around the room with them-
Papyrus: SANS!! IS SCAPING!! GEDDIT!!!
-Papyrus pounces on the red dot and hits the leg of a table, the lamp falling off and breaking-
Gaster: GAH!! PAPYRUS!!! MY LAMP!!
Papyrus: Oops... sory.. didnt meen too...
-Gaster gives out an annoyed sigh and picks the lamp up, putting it back on the table-
Gaster: Its alright, Papyrus... just dont let it happen again. Now you, Sans, and Winter go play somewhere else while I clean up the glass from the lightbulb...
-Gaster walks into the kitchen and gets a broom while Sans, Papyrus, and I go upstairs-
-he picks up the laser pointer and examines it-
Gaster: I dont quite see the appeal of this... its just a red dot. Hmm...
-he sets the laser pointer down on the stairs and gets back to sweeping up the glass shards-
-once he finishes, a red dot appears on the wall, causing him to look up-
Gaster: Huh... what is that doing here?
-he scurries over and slaps the dot on the wall with his hand, lifting his hand to see that the dot vanished-
Gaster: Huh?
-the red dot appears on another wall, causing him to scurry over and pounce on it-
-Sans, Papyrus, and I stand on the stairs, laughing as Gaster chases the red dot around the room projected by the pointer we stole-
