This has been building for a while, it was bound to break eventually. Sans’s insecurities here are all things that have either been not-so-subtly implied to him (like here , here , here , here , or here ) or outright said to his face , and he’s also still haunted by how he was used to try and get Papyrus to kill something and how his most recent attempt to protect him ended in total failure . It wears on you. Papyrus has known about his "condition" since they were babybones but had no idea how deep his feelings about it ran.
(Way to not give him emotional issues , Gaster)
Papyrus has indeed had a scar on his neck since he had that neck brace . It may be hard to see at points but it’s there! He still has it in the future , even. And Sans did indeed tell Papyrus not to let Gaster get to him , haha. LOT OF BACKLINKS THIS ROUND
Sans is glowing a little bit of Papyrus's color there.

and papyrus is absolutely right. his refusal to fight stops more than half of all genocide runs, meaning he's smarter about it than flowey, undyne, alphys or sans is. and, for those he doesn't stop, they rapidly learn that not only is sans not weak, he is the strongest fighter in the underground, bar none.
papyrus is way smarter than gaster gives him credit for. though i guess he has to say that, or else he's gonna fall for them and it's way too late for that.

Damn, you always manage to make me relate to real life with these..
In the one where Papyrus had his breakdown, I commented about how Papyrus channels stress like my sister- seems fine, happy, and strong through the tough stuff, but has a break down over little things (the puzzle cube in that comic) that isn't even about the little thing, but just all of the held-in stress and emotions that have been built up finally cascading out in one go.
This time, with Sans's break down, he reminds me more of myself (ironically, given that Papyrus's breakdown was like my sister): I can get upset over something that I perceive as my fault very easily- even if it wasn't my fault or it was due to unavoidable circumstances. I'm always aware of my own shortcomings and what I am incapable of doing because I have a very poor sense of self-image (in high school, I got to the point where I was constantly insulting and putting myself down to the point that I laughed at bullies who tried to insult me and told them, honestly, that "I insult myself better than that."), but most of the time it isn't a problem. When it becomes a problem is when I mess something up or my inability to do something results in someone I care about being hurt.
An example of the latter would be when I was unable to tell that a friend of mine (whom I had known for 10 years) had been slowly stealing things from me whenever he came over to visit- this resulted in him breaking into my house while my family was away and stealing all of our video game consoles, several games, any cash that was reachable, my dad's gun and medication he needed, and several old pieces of jewelry that my mom received as gifts and hand-me-downs from relatives over the years (many of which are no longer with us anymore). I felt horribly guilty for not noticing my former friend's escalating behavior and how it resulted in my family, particularly my mother, losing many irreplaceable and beloved possessions. Even now, when I look at something that reminds me of that event (something of his he left here, something that got replaced, an old picture of us, etc.), I start to feel like the biggest idiot in the world and I just want to cry and scream about how worthless I am.
Those sorts of breakdowns are typical for me and it's unlikely for me to be able to stop on my own because, honestly, I'm just incapable of seeing my own worth- especially when I'm in the middle of a breakdown. If no one's around, then I'll eventually burn myself out on the crying part and just be depressed and feel like my insides are made of lead for a long time (sometimes several days if no one notices how I'm feeling). The only way I get over it is with the help of someone I care about who reminds me of my own self-worth with loving, honest words. This role is usually filled by either my sister- who for some reason thinks the world of me and is always supportive of me just like I am for her- or by my fiance- who has been my friend since we were in fifth grade and my significant other for 8 years now and knows me better than I know myself most of the time.
So, I really relate to Sans in this comic- when you feel useless, it's hard to get out of that mindset without someone who knows and loves you enough to tell you with %100 conviction and honesty that you DO matter and that you're NOT worthless. But, when you do have someone who can do that for you, they quickly become the greatest treasure in your life and someone that you always cherish, no matter what mood either of you are in.

Heh, it's kinda interesting really...
Papyrus is tortured on a physical level due to his substantial HP, and Sans is tortured on an emotional level due to the fact that he thinks himself worthless with Gaster's distasteful remarks.
Two equally devastating forms of torture, yet quite different from one another.

and both absolutely incorrect about them. papyrus is smarter than anyone suspects, being totally correct about kindness being a solid way to stop an anomalous time-eating murderer, and sans can paint the walls with anyone who would care to dance. it's just that they're both children here, and thus no match for gaster.

They already know there's an outside and a "down here" where they currently are, and Gaster probably referred to the outside as the underworld once or twice, so they picked it up from him.

he's slipped in the past, might have interchangeably referred to "the world" and "the underground", so sans and papyrus think they're the same thing. which makes this even better, since it means pap's telling sans that he's the strongest monster in the whole wide world.
