This has been building for a while, it was bound to break eventually. Sans’s insecurities here are all things that have either been not-so-subtly implied to him (like here , here , here , here , or here ) or outright said to his face , and he’s also still haunted by how he was used to try and get Papyrus to kill something and how his most recent attempt to protect him ended in total failure . It wears on you. Papyrus has known about his "condition" since they were babybones but had no idea how deep his feelings about it ran.

Papyrus has indeed had a scar on his neck since he had that neck brace . It may be hard to see at points but it’s there! He still has it in the future , even. And Sans did indeed tell Papyrus not to let Gaster get to him , haha. LOT OF BACKLINKS THIS ROUND
Sans is glowing a little bit of Papyrus's color there.

I find this very relatable (especially from Sans POV). I deal with depression and anxiety daily, and there are times I've felt the same way. So actually reading this comic speaks to me in a good way. Thanks for posting this as it makes me feel better.

Hmmm, here is a question for everyone, which brother do you think would be the first to completely break in this environment. Papyrus, the innocent, hopeful brother who keeps thinking it would get better. Or sans, the cynic who basically is expecting the worst outcome, but seems to be on the edge already?
Think about this REALLY carefully, don't say "it's going to be Paps or sans because!!" you gotta think about it.

You know, I think they'd both go down together as brothers either way. Hope can last for a while, but can be broken easily if there is no future to be seen beyond the present. Expecting the worst outcome speaks for itself but being on edge all the time could eventually burn someone out. I think that if Papyrus lost Sans, hope or not, he wouldn't want to go on without him. I think Sans would feel the same way.
