In Equestria there is magic and like everyother thing in life there is something bad about it and I can imagine that this scary ass thing decided to possess Pinkie Pie and then claim that it is her true friend and that it finally consumed her after sometime so that this and Pinkie became one and the same, it probably won because Pinkie didn't want anyone to think she has serious problems so she didn't tell Twilight who had a large chance to help her since she is one unicorn and a damn strong one too.
(I know ''Cupcakes'' isn't canon and don't wish it was either but I think what I wrote above can pass as an okay explanation.)
