Its simply a matter of taking three pictures. One of him holding his head in the pose he used for the face. One of himself he used for the head and body, and a picture of a skull. Then you take the picture of him holding his face, use the marquee tool and select the area you want to cut out. Then you copy it into the picture used for the body and head, on a seperate layer. Then you open the image of the skull, and cut out the piece you want to show beneath the skin and copy it into the same picture on another layer.

Take the layer of the skull, select the piece and feather out the marquee and paint it white on a seperate layer, then you will use either a custom gradient or manually color it to show depth in the bends around the face, and make it slightly translucent using the transparency slider. This should take care of the cut face showing the skull, and dont worry about the coloring, it will be fixed in the latter steps.

Take the layer containing the face and use the erase tool to clean up the edges and make it look like the same piece of skin that is cut out. ( I dont like the way the artist kept it a perfect cut, makes it look to manufactured.) It has to match the edging you put around the skull piece, to convey a realistic look. now use the move tool to place it where you want it. Preferably partially infront of the skull piece to show depth.

Now for the coloring. If you are using photoshop, which I hope you are, select the Image menu, then Adjustments and finally Hue/Saturation, and select the Colorize option and choose a color you like and do the same with all the layers.

It also helps that you use the highest mega-pixel option you camera has for the shots, so that you can enlarge or shrink them and not lose detail due to compression or stretching.

Might want to mention this as well, it works well to take all three pictures in the same lighting, so you dont have to throw in filtering effects to try and get a synthetic lighting, which most often looks to fake.
