The bloodspatter is added afterwards in photoshop (that's the only thing I did, except of course color & contrast)
I wasn't too happy with them either (I used the same bloody texture on "on the operating table" where it looked much more realistic) but everyone kept telling me "the blood looks fine! upload it already!"
I still have the ps-file, that's how unsure I am about the end result 
Altough, the image doesn't look quite as "appealing" without the spatter *sigh*
I have a simular image (realise) in my gallery. That one doesn't have blood spatter. It has raindrops on a window ^_^ (yes, those were real... actually, I sprayed the windows myself, people were looking "wtf")

Haha!! I know the feeling! I remember having someone hold a hose on a balcony while I stood with an umbrella photographing a girl in the "rain".
Could tell the bloodsplatter was added afterwards, and even though the other image had distortion (raindrops), I think the blood takes it away from "meaningful photos with meaningful things writting on them", and turns it more into photomanipulation/horror.
You could always, instead of using brushes, use real fake blood. Or even get him to put his fingers in some dirt, to make his hands have the same grungy effect the blood creates.
Either way, I think this is a really great concept and the lines are really capturing. The Realise one hell made my head implode lol.

Me & the same model are planning a shoot with real blood tough. Just need to find a location where I can afford to work messy like that
