I bet this is approximately how Frisk starts to look during the Genocide Run - while the in-game sprite remains unchanged, several characters comment on Frisk's unsettling apperance, mentioning that they "shamble" around like a zombie and are always covered in dust. In the end, Asgore doesn't even recognize them as a human.
