Indeed. It's a critical point by the 11th's era. And the thing is, he is misunderstanding the situation.

I mean, if you want to sum up 11’s initial, general attitude, it is: "I am a bad person and I hate me; but you are worse and threatening humans, so fuck you". 


And he is right; the Daleks, for instance, deserve to be extremely hated. But on the other hand, while he is I believe, the most guilt-ridden and self-loathing Doctor, that is mainly for the things he has failed to do, his mistakes, the people he has failed to protect and the lives he has taken. He does initially believe however, that his attitude towards his enemies is righteous and he doesn't even care if it is when innocents or people he loves are in danger. He considers it natural for them to hate him, it is natural, he would be offended if they didn't. What he doesn't realise is how much they also fear him, how insanely destructive and dangerous he appears to them, and to other people too. And yes, he has started getting glimpses of it and using it in his favor to bluff and avoid confrontations, but he still doesn't realise how desperate this can make them, how it can lead to extreme measures to take him down, how it can escalate conflicts and hurt the people he loves even more.


In "A Good Man Goes To War" he does finally realise it, thanks to River's speech. The problem isn't that he is evil like he fears and believes, it's that he doesn't appear anymore like he is and should be, because of his extreme, though well-intentioned actions. It's not that he will destroy the Universe if he says his name and has to be stopped; the fact that the Question exists is not his fault after all. It's that he inspires so much fear and is so dangerous, and that causes them to think that such extreme and destructive measures are necessary to take him down.

[I mean that was the whole point of the Pandorica plot too. It's not that his enemies form an Alliance of Doom to trap him, that could occur to them as an idea at some point anyway ("Yo, what if we, like, teamed up with the Daleks?" "The Daleks!? Why?" "Well, we could, you know, defeat the Doctor." "...Sweet!"). It's the fact that they find so many willing to co-operate, so many races, some of which aren't even always villains, find it completely plausible that he is willing to or capable of destroying the universe.
(The Daleks are like: “Shit, if he destroys it we can’t conquer it!” “Dammit, that’s our job! He just wants to piss us off, doesn’t he?”)]


He shouldn't inspire so much dread because that is only the start of a possible greater darkness. He realises that while he hates himself because of his past -which in turn causes his violent actions, because he can't bear to let anyone else die on his watch, it's his future he should be worried about, what he could become (not that he lets go of his past guilt, obviously). And he tries to learn from that and change it, by starting to question his own actions, a process that continues with 12, though of course, they both still mess up spectacularly at times.


And consequently, 12 doubts whether he is a good man. 11 hated himself, 12 justifiably fears him.
