You open Photoshop/Painter/whatever and start sketching. After many time (months, years...) of training, you (hopefully) will be doing something like this.
I used to wonder the same thing. Now I see it's just a matter of training, like anything else.

The blue backlight is really good, but what is a lonely christmas tree doing in the lower left corner of the building? It's the only vegetation in the picture and it looks wery odd.
There's something with the perspective in the road that doesn't work, and the upper right corner of the picture is quite dull, make the clouds explode with magic light, that would be awesome.
