Hello Isaac fans, this is round 2! This time around I made 30 small fan art pieces for you to imagine what they would do in the game!I've got to say you made up some cool stuff the first time but now ive gone out and found some real mythes and legends for inspiration!
If you look up some of the passives/items/familiars names you might find that some of them really exists ...as mythical legends. ^^
But maby it'll help you come up with some cool ideas for what these things can do ingame

Warpzone: if Isaac is lowered by 75% of his max health in one room, it will teleport him to the secret room, without any bosses, and he now has a 15% chance to get to the I AM ERROR room when teleporting.

disruption: shoots a ways off than the actual body, out of thin air, works well with brimstone so you don't always take hits while charging

The black heart will show up in Rebirth, and some of the items are in the Community Remix! What were your items supposed to do? I can see the Chesthound being able to carry a single active item inside it (idk how you'd take out the item or put it in) and the item can't recharge while it's inside it, not even the Candle.

Disruption: Isaac’s tears have a 5% chance to fire surrounded by television static. Any non-Boss enemy damaged by a static tear is removed from the current room and teleported to a random combat-room on the current floor.
Warpzone: Anytime Isaac completes a room without taking damage, there is a chance one of the exit doors will be replaced with a warp-zone portal that acts as a one-time use of teleport.
Historian: All ‘book’ spacebar items recharge one room faster. Drops a random ‘book’ item upon collection.
Corruption: Rare item. Grants a plus 2 bonus to damage and turns Isaac’s tears purple.
Void: Tears that strike Isaac have a small chance to be absorbed and deal no damage. Chance increases with luck.
Oni Mask: Upon entering a room, enemies that would normally rush Isaac will remain stationary for a brief period before attacking.
Error’s Beard: Highly increased chance of teleporting resulting in a trip to the ‘I am Error’ room.
Book of the Dead: Reduces speed. Every time Isaac kills a non-Boss enemy, it has a small chance of resurrecting immediately as a friendly entity until the end of the room.
Curse of Nue: Grants Isaac a fixed snake familiar. Rather than following Isaac, the snake clings to Isaac and poisons foes that wander too close.
Divine Calling: Hugely increases the chance of an Angel Room spawning at the end of a floor if Isaac collects no items before killing the boss.
Fountain of Youth: Grants temporary all-stats up on the room boost for the room in which it is used. Has a small chance of hurting Isaac when used, but also has a small chance to heal him.
Black Heart: Trinket. While Isaac holds this trinket, he moves slightly slower, but his tears have the same slowing effect as Little Gish.
Fake Dollar: One-Use Item. Can be used in any shop to make all items in the shop free – but also triggers a fight with Greed before they can be collected. The more money the player has when the Fake Dollar is used, the higher the chance that Greed will be Super Greed.
Book of Despair: Every enemy Isaac kills in the current room has a chance to drop a heart or a soul heart. Bosses have a high chance to drop an eternal heart.
Crackerjackers Blade: When used, Grants Isaac an extended Mom’s Knife style sword for the current room only.
Enigma Bomb: Similar to 3 Dollar Bill, any bombs placed will have a random bomb effect attached to them. Gives +5 bombs.
Tide Jewels: Closes all chests in the current room and fills them with new stock. Can be used multiple times on the same chests if enough charges are available.
Lust’s Flute: When used, summons a random Sin to aid Isaac for the current room. Using the flute against another sin will cause it to upgrade to its more powerful alternative. Using the flute against a sin that is already a Super Sin will cause a second Super Sin to appear.
Apple of Discord: Causes all enemies to treat each-other as enemies for the current room; this includes Bosses who will attack any minions they spawn. Isaac will still be targeted if he is closer.
Krampus’s Horn: Causes Isaac to fire a brimstone shot in all four cardinal directions. While Isaac holds this item, he cannot enter an Angel room.
Chesthound: Drops a random chest every few rooms.
Big Chubby: The higher Isaac’s damage is, the bigger ‘Big Chubby’ gets. As he gets bigger, he gets slower and deals more damage. Attacks similar to Pin and Scolex.
Zombie Baby: Low-Damage familiar. Has a small chance to resurrect enemies killed for the current room only to fight by Isaac’s side.
Little Hera: Has a chance to cause enemies to reverse direction for a few seconds upon dealing damage.
Baby Cluster: Randomly changes to a different humanoid familiar every time Isaac enters a new room.
Sacred Egg: Shatters upon Isaac taking damage and is repaired upon entering a new room. Completing a room without taking damage gives a small chance to spawn a soul heart or an eternal heart.
Doppelganger: Has the same stats as Isaac, but no other passive bonuses like poison touch etc. Copies all of Isaac’s actions in a room two seconds after Isaac performs them.
Spirit of Azrael: Upon Isaac getting hit, Azrael spawns several of Death’s ‘scythes’ to attack enemies in the room.
Kasa-Obake: Orbits around Isaac dealing poor damage to enemies. If it kills an enemy, Isaac gains flight for the current room.
Basilisk: Fires a slow-charging, low damage brimstone shot that freezes enemies with the Mom’s contacts effect for a brief period of time.

Firstly, Nice work Brainsause for coming up with such believable icons and extensions to the game! They already feel in-tune with the game
Secondly, Rythmear, you kinda added to this list (and the other) to come up with some really realistic effects of these passive objects! I mean, the two of you could so easily work together to come up with new times for the next game!!!

True, that they've already got it all planned out, but would they ever want some inspiration or ideas they should definitely check you two out. I mean, I could've easily seen these "Isaacs" in the game no questions asked Now it's just to add another few pick-ups and it's all set to go

Holy fishcake! What a list! I had a blast just reading these ideas man. Almoust all your ideas have some kind of unique twist I never thought would fit these items/familiars so well! You, sir, are awesome. Edmund should check this out, we have golden material here for rebirth!

Yes and thanks for commenting on the first one! :3 I like that you made "Original" an actual thing too. I also think you've made the best description for "Growth" and "Goo" so far on the first one.

Disruption: When damaged, 25% chance to activate the shears effect.
Warpzone: Randomly generated points where a player can chose to teleport by walking over, even if the room is not cleared to another randomly located on the floor (includes, special rooms and I am error room but excludes Mob trap, Boss trap and Boss rooms)
Historian: 100% chance for a library every floor (except the chest)
Corruption: Charge only shot, (same speed as brimstone) fires 30 bullets with spread in an arching pattern (chocolate milk will reduce range and # of shots for an incomplete charge and will not multiply damage)
Void: Replaces tears, on fire, head detaches, freely able to move body, enemies attack body but are pulled to head, head can fly over rocks, head is player controlled bomb which explodes on contact, explosion size and radius is that of normal bomb, enemies chase after head explodes, base 5 second recharge on head which reinflates on body
Oni Mask: SPACEBAR, can turn a grey or golden chest into a red chest, can turn beggars into demon beggars, can re-roll the prices of items in the deal in the devil room, can collect loot from self sacrifice rooms without taking damage, can enter or exit curse rooms without taking damage (charge time= 2 rooms)
Error's Beard: Changes the odds to encounter an I am error room to 30%
Book of the Dead:10% chance to activate the Necronomicon effect upon killing an enemy
Curse of Nue: 25% to have a random familiar when entering a room. 5% to have 2 random familiars when entering a room.
Divine Calling: When damaged, 25% chance to summon 8 beams of light
Fountain of Youth: Restores a heart container for every equipped item recievable from a deal with the devil
Black Heart: +2 speed, damage, tears, health. Beggars and angel rooms no longer spawn.
Fake Dollar: Placed in front of coin slot (similar to golden key) can scam one judgment without losing any coins.
Book of Despair: SPACEBAR opens all doors at the cost of 1/2 of a red heart.
Crackerjackers Blade: SPACEBAR, Replaces tears, Isaac swings sword with a cone hitbox with a legth of about 1 tile, knocks back enemies (affected by knockback modifiers), At max red hearts, forms a spinning projectile like a boomerang that launches out and then returns to the player dealing damage to enemies with a recharge of 6 seconds. Spacebar effect, spawn 4 scythes that home in on enemies and are destroyed on contact (recharge, 2 rooms)
Enigma Bomb: When the Enigma bomb explodes, another bomb spawns in it's place with the same exact momentum 1-3 times.
Tide Jewels: TRINKET +2 tears up, thirds tear speed, triples tear duration
Lust's Flute: TRINKET, 60% chance whenever an enemy makes contact with player, enemy runs away for 10
seconds
Apple of Discord: TRINKET A live bomb randomly spawns in an unoccupied tile in each new room with enemies
Krampus's Horn: SPACEBAR, fires lasers in a cross pattern (recharge time= 1 room)
Chesthound: Eats chests, spits out it's contents onto the floor (Includes golden and red chests)
Big Chubby: once every 12 seconds, attacks in a similar attack pattern to scollex
Zombie-Baby: shoots tears, (clone of brother bobby) 5% chance to fire a green bomb
Little Hera: Familiar, fires blood tears for double damage compared to brother bobby with same tears rate
Baby Cluster: fires one bullet with every tear effect from every familiar, excluding zombie and robo babies but fires at 1/6 of the speed of a normal familiar
Sacred Egg: spawns 2 soul hearts and an eternal heart after being laid by a Basilisk
Doppelganger: Similar to able except it does contact damage as much as a cube of meat
Spirit of Azrael: Randomly spawns and occupies a whole room (similar to a beggar). If you place down a judgement card in the room he is in, he will take all of your red hearts and eternal hearts (and if you only have soul hearts, he will take all of them unless you have an eternal heart which he will only take that) and spawn 3 random deal with the devil or angel items and dissapear
Kasa-Obake: SPACEBAR,will put an umbrella in front of Isaac's face, reflecting all shots (excludes green bombs) and dealing damage with those shots for 25 seconds (charges every 3 rooms)
Basilisk: Lays an egg every floor, a golden egg or rarely a sacred egg. After 4 new rooms and the egg breaks and spawns a whole red heart, golden eggs spawn 4 pennies, sacred egg spawns 2 soul hearts and an eternal heart.

I love Corruption. Doppelgänger could attract the attention of one creature per room. Zombie Baby, shoot poisonous tears. Chesthound, give you a random pickup every few rooms. Historian, change your tears to the last shooting enemy you defeated in the previous room, something like that.

PASSIVE
Disruption: The spirit head will go about and collect items and open chests. (will not take powerups, will not give pay arcade machines etc.)
Warp Zone: Enemies that touch you will be teleported to another part of the room.
Historian: Raises chances of a library on the floor.
Corruption: May randomly lash out with tentacles in a small circle around you, damaging enemies they touch.
Void: Randomly teleports you upon completing a room.
Oni Mask: (charge shot) Charges a fireball that pierces through enemies and stones, damaging all it touches.
Errors Beard: Increases chances of finding an error room upon teleportation.
Book of the Dead: Random book effect upon entering room.
Curse of Nue: Turns you into a black cloud when you attempt to fly over pits. You can't shoot in this state.
Divine Calling: Sometimes, when you get damaged, four beams of light will strike down from above (same as if you used the White Horse).
ITEMS:
Fountain of Youth: Heals you completely upon use, but you lose a heart container. Recharge: three rooms.
Black Heart: Heals one full heart and damages all enemies in room slightly. Recharge: six rooms.
Fake Dollar: Allows you to use an arcade machine once for free. Recharge: one room.
Book of Despair: Deals considerable damage to all enemies in room, and buffs tear rate for current room.
Crackerjackers Blade: Switches tears out for the blade. Max melee damage only. Recharge: two rooms.
Enigma Bombs: places a bomb with a side-effect (can A: leave a slowing puddle, B: leave an acid puddle that only damages enemies, C: becomes a super troll bomb that follows the nearest enemy, or you, if there are no enemies, or D: make a mega explosion. Recharge: 0 rooms.
Tide Jewels: Leaves a never-drying puddle of Holy water that damages enemies.
Lust's Flute: Summons lust to hunt down enemies. She disappears afte ten seconds. Recharge: six rooms.
Apple of Discord: Causes a random activated item effect. (like Dead Sea Scrolls, but better.)
Krampus' Horn: Shoots a four-way brimstone beam upon use, and gives the player black wings for the rest of the room. Recharge: six rooms.
FAMILIARS
Chesthound: Jumps around like Monstro to attack enemies. May leave a chest after completition of a room.
Big Chubby: Looks like a molehill orbiting the player, pops out of the ground to attack enemies that passes over it.
Zombie Baby: Follows enemies and deals very little damage to them. If it touches a baby enemy, it transforms them into Zombie Babies too, that'll help you for the remainder of the room.
Little Hera: Orbits the player, shoots enemies. May drop half hearts upon killing enemies.
Baby Cluster: No effect in itself, but spawns a baby familiar in every room. The spawned familiars disappear upon leaving the room.
Sacred Egg: Spawns a half heart, a soul heart, or an eternal heart every ten rooms.
Doppelganger: Acts like a Doppel, only good.
Spirit of Azrael: Simply hvers around and damages enemies a lot. Disappears in boss rooms.
Kasa-Obake: Attracts enemy shots. If hit, it'll fold out and deflect the shot back.
Basilisk: Chases enemies and damages and paralyzes them. It stops chasing an enemy that's paralyzed.

Super cool list! Love it! Most of it makes sense and would probably work fine in the game itself, which is what I like about it
Least creatuve: Book of the dead. What I really liked: Tide Jewels. I have a similar idea for Enigma Bomb but i think yours is better ^^
