The hand looks weird. The one on the left, that is. (If it is indeed a hand, I could be wrong.) I'm not exactly sure what that weird cum-monster lookin' thing to his left is, and I understand that might be part of the stylistic appeal but...eh. I'm not really seeing it.

To answer your Question:
I design a new bullet/icon reply frist in WORD
using numbers to sketch out the rough idea of the
picture and replace the numbers with DA-Icons and
bullets using the search and replace modus.
After that I copy and paste the results in the
DA Journal-Update Section and take a look at
the result by "preview". If there are corrections
to be made, I either do them in the DA_Journal section or
in WORD, whatever comes in handy. This whole cycle is
repeated as often as needed till I'm happy with the
overall outcome.
It usually takes me at least 10-30 minutes
for the small borders and up to 1-3 hours
for the bigger more complicated bullet/icon pictures.
Once the creation is done it's no problem
anymore...
I save my creations in a file and just copy and paste them later on.
