This is a skin/theme for the free software called CAD (CD Art Display) which runs on windows and can be used to control iTunes, winamp, windows media player etc...
Search for CAD or CD Art Display on deviantArt — there's a group and heaps of top quality skins.
There should be info on the CAD group.

Oh and once you have the program and this skin selected, right click it and go to Skin Editor. Then go to Case and Cover and you can lower Opacity to make it as transparent as you like.

It works with iTunes, WMP, winamp and more programs. Once installed, download this skin and unzip it, copy it into the "Skins" folder. Done.
It's free software and skins like this are really easy to make! You MIGHT need to join the site (I can't remember) but I personally guarantee you won't ever get spammed! There's also no ad crap with the program
