Relaxtion is something I find hard to do - my mind is hyperactive and doesn't like doing nothing. You might think that drawing would be a suitable task, but in fact it requires an incredible amount of concentration. The husky in this sketch looks exactly like I do when not doing something hectic - from a distance it looks relaxed, but look closer and you see the concentration on the object in hand.Oh, and I just like drawing Huskies!

The eye does look a little strange but it looked like that in real life. The coat was an interesting one to draw however....

thats quite the nice shadow divisions for the fur, and the ears are in the correct position
....
the only problem I find is in the eye...or maybe the eye-lid is closed.... but anyway, its really good, good work lofty! O_o

Form has to do with the perspective and the proportions used for the person or thing depicted. A bad use of form would be heads that are too large or small, arms or legs that are too thin or too thick, etc. The form in your picture looks really good!
