Here's how it would go...
Vault 49 was an experiment where they would trick people into signing a contract in which they would be lured down into the padded vault which would seal and for 30 days would slowly be filling up with a gass witch would regress the human body mentally and physically. They would be monitored 24/7 with hidden camera's as an experiment which was to test how human beings that where getting younger would interact with each other. After the experiment was a success the overworld or what was above ground was wiped out by nuclear radiation, leaving the team of 10 scientists to care for the regressed test subjects.
But that's just a theory A GA...I think I'm done here.

An extra survival level where you're incontinent and need to change every so often depending on your hydration level/food level. I'd dig that.

well it goes as a female main character finds adult diapers and figures out how to make them but because of the radiation every time she wears them her bladder and bowel control gets worse

Hmm...Sounds cool, but it might need a jumping off point. A reason why she would take them in the first place. Perhaps Nuka Cola Quantum, the rare drink, causes bladder incontinence?
