Belphegor Form is Akuryo 's form accessed from the Belphegor Fuestle and granting him access to the Sinister Greed Tonfa Belphegor Mist. In this form, the Omnilens and Akuryo's entire chest/legs armor turn a shade of light blue called Belphegor Diamond which places Akuryo under the influence of Nemu.

Background
This form is first fought with Tapir Fangire, using his mist to make people sleep and absorb their souls.

- Pandorus Kivat: Akuryo's partner.
- Pandorus Belt: Transformation device that forms when Pandorus is activated.
- Fuestles: Whistle-like devices that activate a function, depending on the Fuestle.
- Belphegor Fuestle: This Fuestle summon Nemu and allows Akuryo to perform his Nemu Slumbering Blow finisher.
Weapons
- Sinister Sloth Tonfa Belphegor Mist: Akuryo's form-changing weapons that turns him into Belphegor Form.

Powers & Abilities
This form has more agility than his other forms. Akuryo can also send himself into people's dream to take down dream-eating enemies.

Finisher
Akuryo inserts the Fuestle to the Pandorus Kivat to activate the finisher.
Sloth Awoken!
Akuryo's finisher is called Somnia Slumbering Blow. When it is performed, day turns to night and the full moon appears while surrounded by the fog. As Akuryo standing or flying, he extended his wings to blow the Fangire with the blast of air. He then fly towards the Fangire; spinning rapidly along with his tonfas to finish off with the corkscrew attack.

Etymology
- "Nemu" comes from the Japanese word for nemu (眠い nemu, "to sleep").
- Somnia is the Latin word for "sleep".
