OKAY EVERYBODY, LOOK WHAT I DID.I went ahead and just shopped the damn thing myself, so now, you have a full, complete, working version of a Trauma Team patient chart.
it was a bitch to make, let me tell you.
this can be used for a variety of things! it can be used as your dA I.D, an OC's medicial chart, fake screenshots, a variety of thingies! I just want you guys to have fun, and gimme my credit. XD
I left it black and white so you peeps could put what color backgrounds and images you like C:

...you know, I should have mentioned this earlier. but for straight lines like that, the line tool is absolutely perfect for making up something like this, no tablet necessary.
you basically set how thick you want the lines to be in the little 'weight' box, and click and drag to create lines however long or facing which ever way you want; it's really nice.
I don't know why it didn't occur to me until now, I guess your comments just spurred it! XD
