He looks like one of the people of Innsmouth, Mass. in mid-metamorphosis. There was considerable inter-marriage with the Deep Ones, an amphibian species of Cthulhu-worshipping beings. The advantages include breathing water, eternal life and having your prayers (to Cthulhu) answered. Of course, they are quite reclusive, misanthropic and have a weird language predating humanity by geological ages ! They mean us ALL harm !

Once the gills appear and the ears come off, he'll be ready to leave Innsmouth, swim out to Devil's Reef and dive down to Y'ha-nthlei (of which Innsmouth is but a front !)
