My guess would be run ins with dubbing because the voice actors have other more pressing engagements, like for example Ichigo's voice actor, in 2008, was busy assisting with the production of Tales of Symphonia Dawn of the New World as Emil Castagnier (the main character)

Most people generally do figure it out at that point, but I can never be too sure. Some people suspect it at that point, but don't confirm their suspicions until later.
And yeah Grimmjow's ability to be able to do what he wants is pretty damn awesome. Why can't Soul Society have a captain like that?

That's the thing you actually have to upload the document from your computer and then you can edit it.
You could do it either way, I usually do that and the A/N within the document and then add the lines during editing
