There's an anime called Baka and Test. One of the characters is a boy who looks like a girl. His name is Hideyoshi and he got his own restroom since he couldn't go to the girl's and the boys didn't really want him go in with them. He also wears girls clothes and his chest is always covered even though he's a guy.

It's a reference to the Anime Baka to Test. There's a character named Hideyoshi that claims to be a boy, but looks like a girl in every conceivable way. What's even more confusing is that Hideyoshi sounds like a preteen boy, making it even harder to distinguish his gender.

There are voices that are quite hard to tell and lot of times people take it the way they want to. Some decide on female while others decide on male. This is coming from first hand experience.
