There's one thing in Otto's favor, he's still breathing. Β If Tom didn't need him, he'd be dead as a door nail.
From the limited information we've gotten from his childhood I'd say Otto's on the ragged edge of a total panic meltdown. Β If he does come unhinged, he's got enough weight behind him to make driving the van difficult for Tom, if Blue adds her mass (which I think might be a lot more than she appears), they both might be able to flip the van over.

From the placement of everyone in Panel 3, it actually wouldn't be that hard to throw enough weight quickly to the right to topple the van. Β Especially in a sharp right turn.
