Note: For this list, I'm looking at characters, who are tactically and/or instinctively insane, like The Joker.
Rules for this one:
1) Some of these characters can be ones that were redeemed later on, but during their "time in the spotlight," as great villains.
2) No eldritch Abominations, like Mephiles the Dark, or Elder Princess Shroob, but I will make an exception if these said characters displayed something a human is like.
3) The Insane part is main thing for these characters, as long as they are Tactically, and sometimes Instinctively, insane (like 85% Tactical, & 15% Instinctive).
