Amethyst is a special case. As the only gem who consistently eats human food, she's created a stomach for herself at this point to contain it all; if she didn't have a stomach, all of the chewed up food would just slosh around inside of her, making it more difficult for her to move.
Think of it this way: Since gem make their own bodies, they would need to create their own organs, but since they don't need organs... they just don't make them.
