Because Papyrus made up the name, and didn't know what his real name was. Sans adopted the name that Papyrus called him by, and abandoned whatever name he had before.

I headcanon for a headcanon that his parents abandoned him when he was a baby. I still take the idea of Sans being adopted. Since he and Papyrus don't look the same. There is a headcanon for them being twins too. AAA I just can't pick one.

Pappy decided to refer to him as sans until they knew his real name - he couldn't tell them 'cause he was asleep - and than when they asked what his name was he chose to keep sans rather than say his real name.

i think its just the separation between his scalp and jaw; to have a skeleton with a single bone for a head would technically mean he couldn't open his mouth
it's not something i've seen taken into consideration before, but it makes perfect sense, especially as the artist has ensured its obvious for their designs of Gaster and Papyrus
