the TMNT-U is sooo cool XD and the idea from =nichan and *Pimpypants are so awesome
so i hope you can read and unterstand it O_o

But i can say yu here also. With color means the necktie.
example my OC Fay have a oranges necktie.
Other ones have red or blue.

Surname is the second name, normally your family's name.
For Example:
If Casey did this ID Card as a student of TMNT-U, he would fill the 'Name' space with his name, which is Casey, and the 'Surname' space with his family's name, which is Jones.
