Well, its a .psd file that contains layer, light and picture information. Have you been working with photoshop?
It's a button and its used for websites.

Okey, +1 for you! You know about layers right? This pds file contains the same stuff as an xcr (or whatever) file that gimp creates when you save a project with layers and all that stuff intact. Just donload the file and edit the text adn your ready to go!

No problems! You can create one but never host it thou. Just learn HTML (isnt that hard), some css and download a text-editor and you should be fine!
Or you can study how the creator made that button and use it as a "how to do" guide
