I like this a lot.
The intensity of his expression is fantastic, and the textures and details look wonderful.
I love the illumination of his skin caused by the glow from his nostrils, eyes and mouth; with the wisps of smoke being a beautiful touch. I also love the ragged shape to his lips, rather than being a nice, smooth line to designate where his mouth is.
Pretty much an all-around fantastic picture.
