The title was completely tongue in cheek. The idea was that it would feature all eleven of the television Doctors, several of the alternative versions of the Doctors from productions like the Dalek Movies from the 1960's, the Stage Play, animated and Curse of Fatal Death as well as several Doctors from fan-produced videos. It was taken from the idea Paul Cornell introduced in both his book Timewyrm: Revelation and his audio play Shadow of the Scourge that the Doctors past incarnations still reside within his mind along with his future and possible future incarnations. So the alternative versions and the fan versions represent possible future incarnations.

I may have stuck the Valeyard in there somewhere. I can't remember

Back in the 1970's there was a stage play written by the then outgoing script editor and series writer Terrance Dicks wrote a play called Doctor Who and the Seven Keys to Doomsday. Originally, Dicks and the theater presenting the play wanted Jon Pertwee to star as the Doctor on stage. Pertwee had just left the series and it was two weeks before Tom Baker would make his debut. Pertwee was unavailable so an actor by the name of Trevor Martin who had appeared in the episode the War Games as a Timelord played the Doctor onstage. There was even a regeneration scene staged (without Pertwee) which introduced Martin as an alternate version of the Fourth Doctor.

They really do. In the audio Shadow of the Scourge the Seventh Doctor explains to Benny that his former (and future)selves stand in the shadows looking judgmentally at him. So I wanted to give the impression of disdain or annoyance on the part of his other selves
