it's a scar, but I don't remember, whether its origins were unveiled in manga/anime.

If this is the same C.C. of the Rebellion, then the origin of the scar is when she got her Code. IIRC, she was stabbed there by the nun who gave her Geass while she was giving her the Code, or something like that.
