Decided to make this, I did this a few years back before I was on dA but I lost the file somehow.

The dotted brown lines are for folding outwards, brown solid lines are for folding inwards, and the black lines you're supposed to cut on. The white tabs are the glue tabs so you can fold it correctly.

I'm terrible at explaining things.

Um, it should work. I would do an example, but I don't feel like wasting color ink, plus my prototype was already a bit annoying for me to do (I don't have the best patience in the world).
